Advice needed for using a hammer activated powder actuated nailer?

We are finishing a basement, and trying to attach the sill plates to concrete floor with a Remington powder actuated nailer. So far, three unsuccessful attempts. The loads did not fire when the pin was struck. We have followed instructions to the letter.

What could be wrong?
edit: Two times, the "misfired" loads showed no indication that they had been struck, and hadn’t slipped out of place. The third time, there was just a slight indentation where the pin had struck, and still no firing.
